To Register:
All the following steps must be completed before your appointment.

To Register

Step 1:

  • Create a unique Username and Password
  • Enter your first and last name
  • Enter ZIP Code and DATE OF BIRTH (MM/DD/YYYY).

Your First Name, Last Name, Zip Code and Date of Birth must match the information provided to the practice when you scheduled your appointment.

  • Enter Your security code – Contact the office for code
  • Enter our unique practice ID: ZXMiYJ
  • Check the Terms and Conditions Checkbox
  • Click Create Account

Step 2:

Select the Question 1 – Question 3 Security questions and the related answers to your questions.

Click Continue

Step 3:

  • Once logged in, click the Update Information button next to your scheduled appointment to fill out your mandatory patient paperwork.
  • The first page is going to be your Demographic Information. Please review and update the information we have on file. To get to the next page click on the green bubble at the bottom of the page labeled ‘Insurance.’
  • The next page is going to be the Insurance Information. You cannot edit this page but if your insurance has changed please notify us and upload your current insurance card. To get to the next page click on the green bubble labeled ‘Forms.’
  • The third page is going to be your Medical Health History. Please review and update the information we have on file. To finish click on the green bubble labeled ‘finish.’
  • If you are a new patient, do not forget to upload your ID, insurance cards and a photo of concern if applicable.

Step 4: (This does not apply to telemedicine appointments)

Upon arrival please stay in your vehicle and check in using the patient portal or text message 425-677-8867. We will then notify you via text message when to come in for your appointment.
